You may access Ohio driving records by visiting the Ohio Bureau of Motor Vehicles web site: http://www.bmv.ohio.gov/driver_license/abst.htm.  To receive your own three-year driving record of violations and accidents, you must include your first and last name, address, date of birth, social security number and driver license number.  You will need to down load form BMV 1173 – Record Request Form.  Other information may be requested on this form. 

 

To request another person’s record you must also fill out and have notarized form BMV 5008 - Notarized Written Consent Release of Personal Information and submit it with form BMV 1173.  This forms may also be down loaded  at the above mentioned web site.  There is a $8.50 fee for each record requested.  Make your check payable to the Ohio Treasurer.  From the time the request is received, processing will take two to three weeks.

If mailing in a request is easier for you, the address for the Ohio Bureau of Motor Vehicle is P.O. Box 16520 Columbus, Ohio 43216-6520.

If you choose do obtain a record in person you may visit one of the Regional Driver Licensing Reinstatement Centers or the Shipley Customer Service Center in Columbus.  To find the regional center go to http://bmv.ohio.gov/county/reg_off.htm.  The Shipley Center is located at 1970 West Broad Street in Columbus.  Driving directions are found at http://bmv.ohio.gov/county/bmv_csc.htm.  Office hours are Monday through Friday from 8:00 a.m. to 6:00 p.m. and Saturday from 8:00 a.m. to noon.  If you have any questions you my call (614) 752-7500.

You may also obtain Ohio driving records by visiting a local deputy registrar license agency.   In person you may request a record for yourself by providing the above mentioned information and either your driver’s license or an identification card.  Information about another person may be obtained by filling out form BMV 5008 which includes that individuals notarized written consent.
